OPTION 1: Complete the google forms using the links below:

This is a form that we need to keep on file so this will need to be printed and signed by the school or home school family. Complete the waiver form and submit with the appropriate information indicating the following:

***NOTE: Once your classroom student gets their permit, please have them bring it with them to classroom and we will make a copy of it, then send them home with the Behind the Wheel Ideal Drive Time Form for you to fill out and get back to us to schedule them in our next available slot that you request as long as their balance is paid in full.
BEHIND THE WHEEL ONLY FORMS
Behind the Wheel session are scheduled based on availability of the student driver and availability of the instructor. The 12 hours of behind the wheel required by the State cannot be completed in less than a 14 day time period. So we usually schedule drives for 3 weeks for 2 days a week for 2 hours each session or drives for 6 weeks for 1 day a week for 2 hours each session or drives for 4 weeks for 3 hours each session.

1) Behind the Wheel Enrollment Application-Behind the Wheel Only (Submitted all online)
Complete the enrollment application to indicate the driving session for which you want to register. *We are currently about two months out for scheduling any New Behind the Wheel Only students.*
2) A copy of your transcript or report card showing the high school where you took the course and the grade earned showing completion of the Drivers Education classroom coursework.
3) A copy of your valid drivers permit
View a copy of the Behind the Wheel Ideal Drive Time Form to see a preview of our available drive times. This form does not mean that we have all of these times available all of the time, but we will do our best to get you the next available drive time in the slot that you request.

What do I do when I am done with Behind the Wheel?
1) Double check with instructor to make sure you are paid in full prior to the last drive time. No paperwork will be submitted to the State without proper payment.
2) When all of the paperwork is submitted it usually takes 3-5 business days for your certificate of completion to go through which is done electronically and then it tells the DMV that you are set to go for your road test. - Pursuant to Chapter 625 ILCS, Section 6-408.5, the above named student attends this school and has received a passing grade in at least eight (8) courses during the previous two (2) semesters and is, therefore, eligible for private driving instructions.
- Signed by building principal or school registrar.
- If Home School student, use this form in place of the High School Waiver Form: Home School Waiver Form
- Complete the enrollment application to indicate the class session for which you want to register and submit. Please call our Business phone: 309-828-7623 or Business cell phone: 309-287-9478 to check on the availability of the class session. Applicants must be turning 15 by the end of the classroom session for which you are registering.
- Students must be turning 15 by the last class date of the session for which you are registering

- This form explains some of the State, classroom, and behind the wheel laws. It also explains payment and attendance expectations for the classroom hours.
